The following items must be present in each child’s file:

❏ Application (must be complete signed and dated)
❏ Allergy Information (See enrollment application page #3.)
(Note if your child has been diagnosed with a known allergy, please provide the center a treatment plan from your pediatrician.)
❏ Documentation of Receipt of Parent Handbook
❏ Immunization Record, Medical Exam, and {Hearing & Vision (4yr.)
❏ Discipline & Guidance Policy
❏ Financial Agreement
❏ Transportation Policy (only required for school age children)
❏ Updated Polices and Procedure Communications
❏ Photo/Video Permission Form
❏ Infant safe sleep form (only required for children under 12 months of age)
❏ Child Assessment
❏ Copy of Parent’s Photo ID
